The successful candidate will:

  •  Provides supports coordination services to children and adults with developmental disabilities to arrange, manage and provide supervision of supports and services using person-centered planning principles. 
  •  Enhance continuity of services with multiple providers (DHS, schools, vocational services) 
  •  Supports coping skills
  •  Ensures consumers are receiving eligible SSI and Medicaid benefits
  • Competes assessments, diagnosis and evaluations
  • Participates in developing Person-Centered Planning meetings and I.F.P.O.S. treatment plans
  • Acts as consumer advocate
  • Department required to meet productivity standards and maintain a utilization review score of 95% or greater.
  • Maintain and update clinical knowledge by reading, in-service trainings, and other activities as well as provide training and consultation to assigned staff within the divisions. 
  • Knowledge of the theoretical principles and practices relevant to the clinical components of the program and how to apply with client population
  • Ability to establish effective clinical and collaborative relationships with a wide range of persons

Education

Required

Masters or better in Social Work.
